Abstract


The needs for esthetic restoration is ever-increasing nowadays. Especially in the implant restoration, the preservation of the remaining alveolar bone and the gingival tissue including the papilla is very important for the aesthetic restoration. In addition, implant restorations with internal connected type implant are getting increased for the maxillary anterior region recently. The aim of this study is to propose the protocol of implant placement and method of restoration which can minimize esthetic issues. This study is accomplished by surveying multicenter results of treatment applied the internal type of implant in anterior region and evaluating the esthetic outcome of treatment. A total of 195 specimens were selected from 5 different private clinic, were placed and restored in upper and lower anterior and 1st molar missing area and to over age of 20 years male and female between 2005 and 2008. The results of patient¡¯s satisfaction on the final restoration were as follows. 1. There is significant difference depending on implant diameter. Over 4 mm implant in diameter similar to that of 2 mm beneath CEJ of natural tooth showed more favorite results (p<.01). 2. In etiologic factor of extraction, patients were more satisfied in remaining root and fractured tooth but less satisfied in case of loss of alveolar bone and interdental papillae caused by apical lesion, periodontal disease, congenital missing, dental caries, and accidents, with statistical significantly (p<.05). 3. Smoking affected not only the osseointegration of implant at placement but also the esthetic satisfaction. There is significant difference between smoker and non-smoker (p<.05). 4. There is significant difference on the patient's satisfaction among institutes participated in 5 multicenter study (p<.01). It showed high patient's satisfaction on function, esthetics, comfort, paresthesia, and self-cleansing in implant restorations and it can be considered as a one of the predictable treatment methods in the anterior restoration.
